The Heart icon to represent a Toon's Health.
Health is a mechanic in Dandy's World. Health functions using Hearts, which can be viewed in the lower left corner of the screen. At the start of a run, you will gain the maximum number of Hearts, and once depleted, your Toon will die and you won't be able to play for the rest of the run. At this point, you can either spectate others or go back to the lobby to play again.
Regular Toons have a maximum of 3 Hearts, while Main Characters only have a maximum of 2 Hearts. However,
Soulvester is an exception, because his Passive Ability gives him one extra heart, being a total of 4 hearts.
Losing Hearts
One Heart is lost every time a Twisted strikes you in any way, unless a Lethal Twisted hits you (
Twisted Dandy and
Twisted Dyle), where all Hearts are lost. The only other way to lose Hearts is by playing as
Cosmo or
Ginger and using your ability, which sacrifices one of your own Hearts to heal a selected teammate.
Not reaching the Elevator before the Panic Mode timer runs out ignores the Health system and instantly kills any Toon outside the elevator when it closes.
Restoring Hearts
There are currently 7 ways to gain Hearts:
Bandage (Restores one heart for the user; single use.)
- Can be picked up from the floor by
Bassie and given to players, found on the floor, obtained using
Gigi, or buying them for 60 tapes (54 with Frugal) and (30 with
Dandy Plush) at Dandy's Shop.
- Can be picked up from the floor by
Health Kit (Fully restores Health to the maximum number of Hearts for the user; single use.)
- Can be picked up by Bassie and given to players, obtained using Gigi, or buying them for 100 Tapes (90 with
Frugal and 50 Tapes with Dandy's Plush) at Dandy's Shop.
- Can be picked up by Bassie and given to players, obtained using Gigi, or buying them for 100 Tapes (90 with
First Aid/Medical Attention Card (Restores one heart to all Toons that are currently alive and missing a Heart.)
Teagan's Active Ability,
Tea Time (Restores one Heart to the user for the cost of 75 Tapes, with a cooldown of 100s.)
Sprout's Active Ability,
Baked Sweets (Restores one Heart for a selected Toon for the cost of 100 Tapes plus 50 Tapes and 30s of cooldown per extra Sprout on the floor; 100s cooldown.)

Ginger's Active Ability,
Baked with Care! (Deals one Heart of damage to the Ginger and restores one heart for a selected Toon; 100s cooldown.)
Toon-specific Interactions
Multiple Toons can directly interact with hearts:
Poppy, who upon losing a Heart, gains a temporary speed boost (50% speed).
Looey, who gains 20% more Movement Speed for every missing Heart.
Yatta, who drops 4 random candies upon being injured.
Teagan, who can grant themself one Heart for the cost of 75 Tapes.
Cosmo and
Ginger, who can damage themselves to give a targeted Toon one Heart.
Soulvester, who has one extra Heart when compared to other Toons.
Sprout, who can give a heart to a targeted Toon for the cost of 100 Tapes.
Other
Blot's Active Ability,
Blot Jr. (Spawns in a decoy with one heart to attract Twisteds for the cost of 10 Tapes plus 10 Tapes per extra Blot on the floor; 60s cooldown.)
Bassie's Active Ability,
Springful Sharing (Can drop any Item, including Bandages and Health Kits, though this relies on them spawning on Floors or Dandy's Shop, or from
Blind Grab/Lost and Found)
Trinket-specific Interactions
There are a few Trinkets that directly interact with the Health mechanic:
Memory Locket shows the player if there are any healing Items on the current Floor if their Health bar is not full.
Savory Charm protects the player from being killed when they are on 1 Heart, unless the attack was a Lethal Twisted's. This only activates once per run.- Both
Blushy Bat and
Soul Sword activate when the player is hit by a Twisted. Blushy Bat causes the Twisted attack cooldown to increase by 3 seconds, while Soul Sword applies the
Slow IDebuff to the Twisted that attacked them.
Other
Fishing Rod has a chance of giving the player
Bandages at the start of a run.

Trivia
- The only method of surviving being depleted of Health is by equipping the
Savory Charm Trinket. However, attacks from Lethal Twisteds and being caught outside of the Elevator at the end of Panic Mode ignore Savory Charm's effects. - It is currently impossible to lose Hearts while inside the Elevator, unless it is by using
Cosmo's or
Ginger's Ability while the Elevator is open.
- However, in rare cases, Twisteds can actually damage you if you get too close to the entrance.
Pebble used to directly interact with the health system. His previous Passive Ability, Survival Instincts, would give himself a 25% boost in Stamina and Stealth, but it only lasted as long as he was on one Heart.
Soulvester has the highest Health in the game, being 4 Hearts.
